The U.S. Centers for Disease Control and Prevention will host a conference call this Wednesday, May 25, for organizations interested in applying for “Community Transformation Grants” — over $100 million dollars in funding available to community groups who use data and other evidence in work aimed at reducing chronic diseases and health disparities. Specifically, the grants will:
- Support the implementation, evaluation, and dissemination of evidence-based community preventive health activities to reduce chronic disease rates, prevent the development of secondary conditions, address health disparities, and develop a stronger evidence base for effective prevention programming.
- Support evidence and practice-based community and clinical prevention and wellness strategies that will lead to specific, measurable health outcomes to reduce chronic disease rates.
- Support intensive community approaches to reduce risk factors responsible for the leading causes of death and disability and to prevent and control chronic diseases in the nation.
